The Port of Rotterdam recently introduced an incident form intended to report any irregularities during bunker deliveries within the port.

These may include issues related to quantity, quality, safety, or procedural deviations, as well as any other situations that the Port Authority should be aware of.

The port authority said incidents should be reported to enable it to take quantified action to address problems and enhance the reliability and efficiency of bunker operations.

It added reports will help identify technical, procedural, or coordination issues, equipment malfunctions, and dispute causes—information that is essential for continuous improvement.

Note: The Bunker Incident Form can be found here.
